United Healthcare

All services paid for by insurance must have a clinical focus working exclusively toward a medical necessity that meets diagnostic criteria. If services are not medically necessary and session content does not aim toward reducing symptomology of a billable mental health diagnosis, services must be done on a private pay basis.

Whether services are covered in full or in part depends on your current health insurance provider or employee benefits plan. Please contact your insurance provider to verify how your plan compensates for mental/behavioral health services.

It is recommended that these questions be asked to your insurance provider to help determine benefits specific to your plan:

  • Does my health insurance plan include mental/behavioral health benefits?

  • Do I have a deductible? If so, what is it and have I met it yet?

  • Do I have a copay/coinsurance? If so, what is my copay/coinsurance amount?

  • Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?

  • Do I need written approval from my primary care physician in order for mental/behavioral health services to be covered?

  • Can I be reimbursed for paying privately for mental/behavioral health services?

  • Does my mental/behavioral health plan cover telehealth sessions?

For insurance carriers that consider HOPE an "out of network" provider, receipts are provided for you to submit to your insurance company for possible "out of network" reimbursement if you are working with a fully licensed counselor. The amount of reimbursement depends on the requirements of your specific insurance plan.

 Good Faith Estimate Notification

Under the law, health care providers are required to give clients who don’t have or are not using insurance an estimate of the expected charges for health care items and services before those items or services are provided.

You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including mental/behavioral health services. You can request an estimate before you schedule a service, or at any time during treatment. If you do, make sure the health care provider or organization gives you a Good Faith Estimate in writing within 3 business days after you ask. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate and the bill.

For questions or more information about your right to a Good Faith Estimate, or how to dispute a bill, visit www.cms.gov/nosurprises/consumers, email FederalPPDRQuestions@cms.hhs.gov, or call 1-800-985-3059.

HOPE Therapeutic Alliance requires a 24-hour advance notice of cancellation of an appointment, if you are unable to attend. Your card on file will be charged in the event that you late cancel or no-show your scheduled appointment.
